This application form can be downloaded as a PDF (portable document format) document  to be printed, filled manually and mailed in OR, can also be filled online, printed and mailed in. 

DOWNLOADING FORM
In some browsers, you may view a PDF document without downloading it by enabling your browser to launch the Acrobat Reader. Before attempting to set up your browser, be sure that you have the Acrobat Reader properly installed. 

To download the file to your hard drive, please do the following:

Right-click the mouse over the PDF file link and a menu will appear.

In Microsoft Internet Explorer browser, choose "Save Target As ...." 
In Netscape Navigator browser, choose "Save Link As ...." A dialogue box will appear.

Save file to desired location. The file will begin downloading.

A window may appear that says "Download complete." Click on "OK"

Open the Acrobat Reader program. Go to the "File" menu, go down to "Open..." and find the folder you saved the pdf file in. Highlight the filename and click on "Open."


FILLING OUT THE PDF FORM ONLINE:


Once open, you can fill the application form on your PC and print.  DO NOT SAVE FORM in the original name if you intend on keeping the blank form for further use. You can instead make a copy of the original form, save it to another file name and print the copy you have filled in. 

IMPORTANT: You cannot save the data entered in a form using the Save or Save As commands. These commands save the form itself, but not the data entered into the form fields.

To preserve data entered into form fields, you will need the Adobe 4.0 (or higher)  program and not the free Acrobat Reader. If you have the Adobe 4.0 (or higher)  program installed on your machine, you must export the data. 

To export the form data:
Choose File > Export >Form Data
 
This would save the form data in a separate FDF (forms data format) file for use later. The form itself is not saved. Type a filename, and click Save. The newly created file will be in Forms Data Format (FDF) and  will be considerably smaller than the container PDF file. The smaller FDF file is useful for archiving or electronically sharing data. You can also import  the data from this file into another Development Review application pdf form, as fields with the same names.

To import form data from a file:
Choose File > Import > Form Data.
Select a file, and click Select (Windows) or Open (Mac OS).

Note: If you import form data from a form that does not match the form you are importing into, only the form fields that match are updated, and those that do not match are ignored.

To clear out or reset the form in Acrobat Reader:
Choose File > Revert

To clear a form in the browser window:
Do one of the following:

- Exit the Acrobat viewer without saving the file and start again. 
- Highlight and delete the portions you have typed in. 

Clicking the Reload button or the Go Back button, or following a link in a World Wide Web browser window, does not clear a form

Important: There is no undo for this action.
